Az Bodywise accepts some insurance plans. Many of our practitioners are providers with: Multiplan and other plans that are housed beneath the PHCS network, MediShare, ASH. However, provider status does not guarantee coverage. We recommend that you contact your provider to verify your coverage. A super bill can be provided that has the necessary information for you to submit to your insurance company for reimbursement.

Co-pays are due at the time of service. We accept cash, HSAs, check, VISA, Mastercard,  Discover and debit cards with the VISA logo.
